How To Apply

Who Qualifies?
  • To qualify for financial assistance through The “J” Foundation, (“Foundation”) a request must meet the following criteria:
    1. Children and adolescents 17 years if age and under.
    2. All referrals must be made by a social worker, nurse, case worker, or other healthcare provider currently familiar with the child and his or her terminal illness and family situation.
    3. The child must have a diagnosed of life-limiting illness.
    4. The child must be a resident in Pennsylvania, New Jersey, or Delaware.
    5. The child must be dependent on parents or guardians.
    6. The child’s family must be experiencing financial challenges as a direct result of expenses related to the child’s life-limiting illness.

Referrals

Children being referred for assistance must reside in Pennsylvania, New Jersey, or Delaware. Families may not apply to The Foundation directly. Referrals must be made by a healthcare professional (for example, a social worker, case manager, nurse, doctor, or therapist).

Next Steps
  1. Healthcare professionals refer patients and their families to The Foundation with our online application.
  2. Using the online application, referring professionals provide information about the child’s diagnosis, the family’s financial situation, and the type of financial assistance requested.
  3. Requests for assistance are generally processed within 24 to 72 hours.
  4. If the application is approved, funds are sent directly to the vendor.
Types of Financial Assistance provided by The “J” Foundation
  • Celebration of Life expenses
  • Utility bills
  • Mortgage and/or rent
  • Car payments and transportation costs
  • Disability needs (e.g., wheelchair ramp, minor home renovations, etc.)
  • Medical equipment
